Fred Khosravi (Chairman)
Independent

Fred Khosravi is a Silicon Valley medical device entrepreneur who has co-founded multiple companies, including EndoTex, Embolic Protection, Inc. (EPI), both acquired by Boston Scientific Corporation, Confluent Surgical, acquired by Tyco Healthcare, and MarketRx, acquired by Cognizant Corp. Additionally, he is the managing director of a medical device incubator called Incept. In addition to Confluent, EPI, and AccessClosure, Incept has spawned several other novel device companies including Sadra Medical (acquired by Boston Scientific), Ostial Corp., HotSpur, Inc., and Ocular Therapeutix, Inc., where Fred serves as co-founder and Director/Chairman. Fred also served on the boards of Kerberos before its acquisition by FoxHollow Technologies and Advanced Stent Technologies prior to its acquisition by Boston Scientific. He holds a B.S. in mechanical engineering, and graduated summa cum laude, obtaining a M.S. in mechanical engineering from Tennessee Tech University.
Hank Plain
Morgenthaler Ventures

Hank Plain is a Partner at Morgenthaler Ventures and the Vice Chairman of The Foundry, a premiere medical device incubator providing expertise in founding, funding, product development and operational management. Recently, Mr. Plain was on the board and a member of the deal committee that helped successfully negotiate the acquisition of Acclarent by Johnson & Johnson for $785 million in December 2009. In addition, Mr. Plain was Chairman of the Board of Embolic Protection, Inc., helping to negotiate the acquisition of EPI by Boston Scientific in 2001. He was also a director of TransVascular, Inc., which was acquired by Medtronic in 2003. Previously, Mr. Plain was President and CEO of Perclose, during which time he led the company to $100 million in sales and managed the company’s successful IPO and acquisition in 1999 by Abbott Laboratories. Prior to joining Perclose, Mr. Plain was with Eli Lilly and Company for 12 years in various management positions.
Gregory D. Casciaro
President & CEO, AccessClosure, Inc

Gregory D. Casciaro has over 28 years of medical device experience, 23 of those years in the cardiovascular space and leadership positions in both public and private companies. Prior to joining AccessClosure, he was the President and CEO of XTENT Inc, (XTNT), a revolutionary drug-eluting stent platform. Prior to XTENT, Greg was the President and CEO of Orquest Inc., a private company manufacturing and selling bio-therapeutic products to the orthopedic market. Orquest was acquired by DePuy Acromed, a division of Johnson and Johnson. Prior to that, he was President and CEO of General Surgical Innovations (GSII) which was acquired by US Surgical, a division of Tyco (TYC). He has held several senior level positions with Devices for Vascular Intervention, a division of Guidant Corporation (GDT), and North American Instrument Corporation. In addition to his operating roles, Greg is on the Board of Directors for Trireme Medical Inc, and a former Director at Kerberos, acquired by FoxHollow Technologies, and a former Director with AngioDynamics (ANGO). Greg graduated from Marquette University in Milwaukee, WI with a B.A. in Business Administration.
Jeani Delagardelle
New Leaf Venture Partners

Jeani Delagardelle is a Managing Director of New Leaf Venture Partners, a healthcare technology fund established in August 2005, by healthcare technology partners of the Sprout Group. Mrs. Delagardelle joined Sprout in August 2000 and became General Partner in July 2001. Previously, she was a General Partner at Weiss, Peck & Greer Venture Partners where she focused on healthcare investments. Before joining Weiss, Peck & Greer Venture Partners, Mrs. Delagardelle spent 15 years in senior marketing, sales, business development and general manager positions in the healthcare industry. Specifically, she was Vice President of Global Marketing for Target Therapeutics (acquired by Boston Scientific for $1.1B) and spent nine years with the Medi-tech division of Boston Scientific in several senior management roles. Mrs. Delagardelle attended University of California at Irvine (MBA) and Occidental College (A.B.).
Rob Kuhling
ONSET Ventures

Rob Kuhling has experience in a variety of technology marketing, strategic development and management positions. Mr. Kuhling was Director of Marketing at Sun Microsystems, where he spearheaded the company's expansion into the design automation marketplace. He also served as Vice President and Manager of General Electric-Calma's $100 million electronic design automation business and was a team leader for Boston Consulting Group. Mr. Kuhling serves on the boards of several public and private companies, and has led ONSET Ventures' investments in companies like Clarify, Conceptus, Alteon, Packeteer, Embolic Protection, Endotex Interventional Systems and Gadzoox Networks. Mr. Kuhling’s current board memberships include AccessClosure, Apieron Biosystems, Curon Medical and Invarium. Mr. Kuhling received his A.B. from Hamilton College and his MBA from Harvard Business School.
Conrad Wang

Conrad joined Three Arch Partners as an Associate in January 2006. Prior to joining Three Arch Partners Conrad completed his orthopedic surgery training in the Harvard Combined Orthopedic Residency Program. Conrad has a BS in Mechanical Engineering from MIT and an MD from the Harvard-MIT Division of Health Sciences and Technology at Harvard Medical School where he was awarded the Rhone-Poulenc-Rorer Research Fellowship. He also has an MBA from the Harvard Business School where he was named a George F. Baker Scholar for outstanding academic achievement.
L.N. Hopkins, M.D.
State University of New York, Buffalo

Professor and Chairman of the Department of Neurosurgery at the University of Buffalo (SUNY), Dr. Hopkins fostered the creation of the Toshiba Stroke Research Center bringing together neurosurgeons, cardiologists, physicists, and radiologists to study the neurovascular circulation and develop innovative approaches to diagnose, treat and prevent neurological diseases. Dr. Hopkins is a recognized leader in the field of cerebral microvascular - neuroendovascular surgery and is the author of numerous publications on the prevention and treatment of stroke.
Jeff Gold
Longitude Capital

Jeff Gold currently serves as a Partner at Longitude Capital. Prior to joining Longitude he was President & CEO of CryoVascular Systems, Inc. through the completion of its acquisition by Boston Scientific Corp. Prior to joining CryoVascular Systems, he was Executive Vice-President and Chief Operating Officer of CardioThoracic Systems (CTS), a publicly traded company that was acquired by Guidant Corp. in 1999. Previously, Mr. Gold spent over eighteen years with Cordis Corporation, now a Johnson & Johnson company as co-founder and President of Cordis Endovascular Systems. Mr. Gold received his engineering training at Northeastern University in Boston, Mass. and earned his MBA from University of Florida.
Jeffrey C. Lightcap

Jeff Lightcap is a Senior Managing Director at HealthCor Partners, a leading growth-oriented private equity fund which invests across all sectors of healthcare. HealthCor Partners is affiliated with HealthCor Management, a $3.0 billion heathcare-focused hedge fund. Prior to HealthCor Partners, Mr. Lightcap was a Senior Managing Director for over nine years at JLL Partners, a leading middle-market private equity fund. Prior to JLL Partners, Mr. Lightcap was a Managing Director at Merrill Lynch & Co., Inc. in charge of leverage buyout coverage for the mergers and acquisitions group. Prior to joining Merrill Lynch, Mr. Lightcap was a Senior Vice President in the mergers and acquisitions group at Kidder, Peabody & Co. and briefly at Salomon Brothers. Mr. Lightcap received a B.E. in mechanical engineering from the State University of New York at Stony Brook in 1981 and received an MBA from the University of Chicago in 1985.
Sealant Science
The Mynx family of vascular closure devices represents a new approach to vascular closure by using targeted placement of the Mynx Sealant, which is composed of polyethylene glycol (PEG). PEG is a water-soluble, bio-inert, non-thrombogenic polymer known for its well-established safety profile and broad use in the medical field. PEG is used in products such as gel caps and cranial sealing (Duraseal™, Covidien).
The original Mynx Sealant consists of freeze-dried polyethylene glycol (PEG). Immediately upon placement of the Mynx Sealant on the arteriotomy, blood infiltrates the porous structure and the sealant expands to three to four times its size*. When fully expanded, the sealant consists of 95% blood and fluids and 5% PEG sealant.
As blood collects within the porous matrix, the coagulation produces a durable hemostasis and platform for natural vessel healing. Within 30 days, the sealant is absorbed by the body, leaving nothing behind but a healed artery.
Introducing Grip Technology: Grip Technology is a new configuration of the PEG sealant that actively grips the artery. When combined with the original Mynx Sealant, Grip Technology achieves active extravascular hemostasis, dissolving within 30 days and leaving nothing behind but a healed artery.
The MynxGrip Sealant is comprised of two configurations of polyethylene glycol (PEG), the Grip Tip and the original Mynx Sealant.
Grip Technology and the Grip Tip
The Grip Tip is a new configuration of PEG, developed using our proprietary Grip Technology, which allows the sealant to actively adhere to the artery wall.
Once the MynxGrip Sealant enters the tissue tract, the body’s temperature and pH level cause the Grip Tip to soften and interlock with the contours of the vessel wall, effectively gripping the artery and providing active closure.
Simultaneously, the porous structure of the Mynx Sealant absorbs blood and subcutaneous fluids, swelling three to four times its original size and filling the tissue tract.
The Result: A sealant that actively grips the artery while expanding and filling the tissue tract, providing a durable hemostasis and a platform for natural healing.
